testing services accessible and affordable, we offer flexible
payment plans to individuals who need time to pay for their
tests. To have a standard test started, we only require a
nonrefundable partial payment of half of the test cost. From
there, we will do our best to accommodate your payment needs.

insurance. In some cases, your payment may be accepted by your
insurance company. In that instance, please check with your

medical counsel. If your test results are out of range, a staff

Customers have the option of taking a test anonymously or not.
If a customer chooses to take a test anonymously, they may be
required to give a birth date or show their sales receipt to
verify the correct results. |

to ANY LAB TEST NOW®? and it will do everything possible to
protect the privacy of its customers. Customers have the option
of taking a test anonymously and choosing which method to
receive their lab test results (U.S. Mail, Fax, Pick Up).
If a customer chooses to test anonymously, they may be required
to give a birth date or show a sales receipt for verification.
Because ANY LAB TEST NOW®? does not utilize insurance, your
results are not given to your physician and not including on
your medical record.
However, customers may provide the results to their physician if
desired. |
